To complete your registration for the tryouts, we kindly request that you submit the tryout fee of $25.00 via E-Transfer. Please send the payment to the email address ManitobaMagic@gmail.com.

Consent and Release:

I, the undersigned, am the parent/legal guardian of the above-named participant. I hereby give my consent for my child to participate in the Manitoba Magic Basketball Program tryouts.

I understand that participation in basketball involves physical activity and that there is a potential risk of injury. I hereby acknowledge that my child is in good physical condition and has no health issues that would preclude them from participating in these tryouts.

I agree to hold harmless and indemnify the Manitoba Magic Basketball Program, its coaches, volunteers, and any other affiliated personnel from any claims, demands, or causes of action arising out of my child’s participation in the basketball tryouts.

In the event of an emergency, I give permission for the staff to seek medical treatment for my child and agree to cover any associated costs. I understand that every effort will be made to contact me in the event of such an emergency.

I understand that, if selected for the team, my child must be committed to travel to three US tournaments in the months of April, May, and June. I acknowledge and agree to make the necessary arrangements for my child’s participation in these tournaments.

Photographic Release:
I consent to the use of any photographs or videos taken of my child during the tryouts for promotional purposes by the Manitoba Magic Basketball Program.
